Hand, Microsurgery and Brachial Plexus Reconstruction Fellowship

Hand and reconstructive microsurgery is a relatively new subspecialty of Orthopedic Surgery, and its importance is now increasingly recognized. Our institution and department have grown under the able guidance and administration of Dr. Kazuteru Doi, who is a world-renowned hand and reconstructive microsurgeon. We pride ourselves in having the largest number of cases in one of the most sophisticated treatment facilities in this field worldwide. General President of ODGH, Dr. Kazuteru Doi, was appointed as the Organizing Chairman for the 5th congress of the World Society for Reconstructive Microsurgery held in Okinawa in June 2009, with over 700 participants. Vice President of ODGH Dr. Yasunori Hattori, is the President for the 47th Annual Meeting of the Japanese Society for Reconstructive Microsurgery, to be held in Kitakyusyu, Fukuoka in November 2020. This meeting is organized in conjunction with the 5th Congress of the Asia Pacific Federation of Societies for Reconstructive Microsurgery.

The most notable strength of our department is its role in the management of brachial plexus injuries. Our department serves as a tertiary center for reconstructive surgery of brachial plexus injuries, with patients being referred to us from all over Japan. We have successfully restored shoulder, elbow and function using functioning free muscle transfers, even in cases of total root avulsion injuries.

Our hospital serves as a referral center for the entire Yamaguchi and its neighboring prefectures, such as Shimane and Hiroshima, for complicated hand and upper limb trauma. We have adopted the use of arthroscopic assisted reduction for intra-articular distal radius fractures.

In addition to managing trauma, our hospital specializes in reconstructive microsurgery. Annually around 20 cases of finger amputations undergo replantation surgery in our hospital. Forty additional cases requiring free tissue transfer (including reconstruction for skin loss by free flap transfers and the reconstruction of large bone defects by vascularized bone grafts) are undertaken by our department annually. We pride ourselves in achieving a success rate of more than 95% consistently.

A niche specialty we provide is the lymphedema service. We perform an average of 20 cases per year of lymphovenous anastomoses for lymphedema treatment. Combined with our in-house lymphedema therapy services such as lymphedema massage, compression bandaging and physiotherapy, we have achieved effective results in our patients.

Common elective hand and wrist surgery conditions are managed at our hospital. These include treatment of osteoarthritis, rheumatoid arthritis and its complications, compressive neuropathies, tendinopathies, tumors, collagenase injections and surgery for Dupuytren’s contracture, and congenital deformity correction. Arthroplasty for osteoarthritis and rheumatoid arthritis as well as arthroscopic evaluation and treatment of wrist disorders are also performed at our hospital.

The Department of Hand and Reconstructive Microsurgery at ODGH is accredited by the Japanese Society for Surgery of the Hand as a teaching institute for Advanced Clinical Training of Foreign Medical Practitioners and as a teaching institute for advanced clinical training by Ministry of Health, Labour and Welfare of Japan. Our institute is known worldwide for its research and education in upper extremity care and microsurgery. Fully trained Plastic and Orthopedic surgeons from all over the world and Japan have trained as fellows at ODGH for additional training in hand and microsurgery. To date, more than 30 physicians from 15 countries have received the fellowship training at our institute.
